I've never used ImageShack, but you generally need to put the "Direct Link" code from your pics into the "Associated Web Images" boxes on the Design Addict page.
As for the chair, I'll defer to DA's Bertoia aficionados.
Offhand, it looks right to me
(...though I warn you that my familiarity with this chair is pretty limited.)
The frame looks identical to the one below, though some are also pictured with shock mounts.
