Herman Miller chair ID - low back shell?

Hello everyone-
I just picked up this Herman Miller shell chair. I've never seen one like it and I've been unable to find any info on it. It's a fiberglass shell chair, but is has a low back to it. It doesn't have the proportions of any other shell I've seen. It has a Summit stamp. Any information would be appreciated! What year? What is the sticker all about?

Bases can be the original or any of the typical HM fiberglass shell chairs like in your example the later H-base with plastic glides.

Great! Thanks for the info. Can I assume by the last two digits under the seat that this chair was produced in 1952?

Probably not. If the base with white plastic glides is any indication, this chair was produced in 1957 or later. And the raised "HERMAN MILLER" letters started in around 1960. So some time after 1960. Those numbers are not date related.
Still a very nice find!

Also as stated in the Eames office link, the La Fonda chairs are from 1961.
The rubber edge and the fiberglass type are typical contract HM.
