Herman Miller Fiberglass Chair Date and Correct Base Identification - Help!

Hello All, I am trying to find out when my Herman Miller fiberglass chair was made and what the correct base would have been. See photos. Not sure the specific color name but its an ivory color.
The original paper label is in tact and measures 3" long x 2" Tall... Is this the small or large label?
The label has patents from the late 40s on it... Does this help date the piece?
The label was also stamped in red "7 6 2" Does this date it from 1962?
The body of the chair is embossed with the Herman miller, which I've read dates it from 1960 or later? The chair is also embossed with the crescent C with a star which means it was manufactured in Cincinatti Milicron.
The body is also stamped with " 928 d" (cant makeout the last #/letter). Does anyone know what this means?
Lastly, I am most confused with the correct base for this chair. I purchased it without one and on the mounts they are embossed with "C N". Does anyone know what this means for the correct base for my chair?
Thanks for your help everyone, if you know any answers to my questions above or better info on the research I've done that would be great!

impressions on mounts indicate standard H base
